We are open Monday and Thursday 8 am-7 pm, Tuesday, Wednesday, and Friday 8 am-5 pm. 8 am-8:45 am is for blood draws only.

We accept most major health insurance plans. Call us to verify your insurance. We accept Medicaid and Medicare. We will be happy to bill any secondary insurances. The patient will be responsible for any balance due. We will work with under/uninsured patients on a case by case basis. We also accept major credit cards and cash. Co-pays and fees are due at time of service.

During your first visit to our office, we will ask that you arrive 15 minutes early to complete paperwork on your medical history. If you are transferring from another practice, we will contact your former doctor to obtain copies of your records. At your first appointment, your provider will answer questions, concerns and perform a physical examination.
